(Time Slip Instructions)

TIME SHEET INSTRUCTIONS:

REP work weeks begin on Monday and end on Sunday. All time sheets must be completed and signed by an on-site supervisor or by someone authorized by the client. The time sheet must be received at the payroll office of REP by Tuesday of the following week. Payroll is processed from Tuesday through Thursday for the previous week so that payroll checks are available on Friday by noon. Checks are available for pick up from noon through 5:00 p.m. and if they are not picked up then the remaining checks are mailed Friday night.

We will accept FAXED time sheets ONLY IF THEY ARE SIGNED BY YOUR SUPERVISOR. Please notify us immediately if you have trouble getting your time sheet signed. In the event that you work solo or there is no one available at the end of your assignment to sign the time sheet, take a copy with you, but leave a copy behind with a note attached asking that it be signed and faxed to REP.

********PROPER TIME SHEET PROCEDURE********

  • 1: Your full name (with middle initial)
  • 2: Check the box, only if your address has changed.
  • 3: Sunday's date at the end of your work week
  • 4: Enter time you arrived at work (round hours to nearest quarter hour)
  • 5: Enter time you are finished for the day
  • 6: Lunch time: Lunch time must be taken unless specifically authorized by your supervisor
  • 7: Put the total hours you worked for that day, minus your lunch
  • 9: Add all your hours for the week and place total here.
  • 10: Note any overtime (Over 40 hours per week)
  • 11: Name of the company or the location where you are working
  • 12: Print your manager’s name, (the person you are reporting to)
  • 13: Have your supervisor sign here, (If the time sheet is not signed we can’t ensure a timely paycheck)
  • 14: The title of your supervisor that signed the time slip
  • 15: Date that time sheet was completed.

    *Make a copy and leave with the company you worked for
